TRAINING NOMINATION/REGISTRATION FORM

NOTE THE FOLLOWING MANDATORY CONDITIONS:

1. Training Duration is Six (6) months.
2. NO Refund of money after payment.
3. Withdrawal from training is at your own risk.
4. NDT Certification on each NDT method shall be based on the successful completion of the Course and passing the Examination with a pass mark of 70% for General and Specific part, and 80% for the Practical aspect of the Course.
5. For failed aspect of the NDT examination, a resit fee of NGN 40,000 only shall be charged to resit the examination for each NDT method.
6. The following shall be provided by the trainee:
   • Scientific Calculator
   • Mathematical set
   • 30cm Transparent Ruler
   • Notebook (Big size)
   • Passport Photograph
   • Appropriate PPE where required for Practical and Field Activity
